SummerBreeze Fest got announced today, which is ANOTHER European summer fest with a kick ass lineup. Any mixture of bands on there would have fit perfectly with the Mayhem Fest scene, in my opinion (not too obscure, but not Warped style either).
So I have a serious question: Why are these fests so common in Europe, but not in the U.S.? Judging by the Mayhem Fest reaction, there is an obvious demand for better and larger metal festivals in the states. So why aren't they happening?
We have the fans. We have the demand. We have the space. So why aren't these fests happening?
I think it has to do with bad business decisions, more than lack of demand or support from bands. And if that's the case, why do you think festival companies aren't trying to change for the better, and replicate what is happening in Europe?
I think there's generally just more support for the arts in Europe, so there are more people who are open to going to these festivals and supporting these underground bands on euro fests. Also, the us has bands coming to your all the time. Usually these festivals are the only appearance a certain group will make in that country, so there's that.
I think exclusiveness of certain artists like Slap said is the key factor. Even if Mayhem got good bands, none of them would be a special or exclusive thing. It'd be bands that tour the states all the time.
Going to European Festivals means putting up with Europeans. I'll take headlining tours year-round over a few good festivals in the summer. Better shows, longer sets, and much cheaper.

Geography probably also plays a part. They aren't as spread out as we are in America and are better suited to travel easier(high speed trains etc). You have to cover a lot more ground to reach the same amount of people in America.

Metal is beyond alive and well in Europe. Its just down in the U.S. when it comes to current touring festivals.
If metal's success in the mainstream is your concern you're doing it wrong.
