The album is known for its bloody cover art, depicting a Barbie doll on a crucifix and a knife impaling a bible, which nearly prevented Staind from gaining a record deal.
In 1997, Fred Durst tried to prevent the band for opening for Limp Bizkit in Hartford, Connecticut solely due to the disturbing album cover. He was unsuccessful and, upon hearing the band's performance, changed his view of the band, eventually signing them to Flip/Elektra Records.
